Community Services Forms

There are 4 categories of volunteer forms available:  the medical first responder package, the therapy dog package, the youth volunteer package and the request for volunteer event coverage form.  Completed therapy dog package or youth volunteer package must be sent into Provincial Headquarters to St. John Ambulance, 2625 - 3rd Ave, Regina, SK, S4T 0C8.  Completed medical first responder packages and the request for volunteer coverage form must be sent in to the location closest to where you live via fax, email, mail or in person. 

A listing of the locations can be found in the "About Us" section under "Locations".  Please be aware that all criminal record check forms must be taken to your local police station.  The letter you receive from the police station, is the original form that is must be dropped off in person or by mail to the appropriate location.  Always confirm that all forms have been received by our offices.

Request for Volunteer Medical First Responder Event Coverage
Our MFR volunteers cover many events with the majority of their funding for uniforms, mobile posts, supplies, etc resulting from donations for event coverage.  These events include local sports tournaments, hockey games, Rider games,etc.  If you are interested in medical first responder volunteer coverage at your special event, please fill out the following form and send it in to the SJA office nearest the location of the event.  To see a listing of our offices, please click here.
